I do kinda agree that shortening the Nanako arc would have made for an easier read, but what loses in comfort it earns it back in impact, which is worth more in my book.

The fetus thing is a recurring motif. I don't what it's meant to symbolize, other than Nakoshi being a manchild. I think it's worth noting he does it even before losing his job, so presumably it's a lifelong habit of his.
Other than that it's also a pseudoscientific explanation for why drilly brain hole gives soulreading capabilities.
