Pretty much this. As a (learning) artist, I've actually tried to draw average girls. And most of the time they just come out looking like ara ara types or a girl with unkempt hair. Basically, the Hex Maniac look.

Because anime features are already so simplified. In order to make them more "ugly" I have to add more details like bags under eyes or asymmetrical eyes or a bigger nose or a stronger jaw line, etc. And even then, someone will find that attractive. This is why so many of the "ugly" girls in anime tend to just have a few altered features. Heck, most of the time they just draw a girl with a mole and messy hair and that's suppose to be "ugly."
