It's not a fetish. Well, okay, it is, but far more people enjoy gore non-sexually than sexually. Gore activates a primitive part of your brain to do with predator-prey dynamics. Gore shows that things are really really really serious, which gives the situation more dramatic tension. That's why even battle shonen where people can take grizzly wounds are more compelling than ones where they don't even bleed. Even Dragonball became worse when it dropped the rare but impactful gorey moments.
