I'm not sure they'd get along very well. Yoshimune was all for austerity, while Arikoto liked coming up with ways to make life more comfortable for everyone. In the very least he would disapprove of Hisamichi's methods, which could turn ugly.
Sugishita was a pretty good match for Yoshimune. The only thing he did wrong was not noticing what was going on between the sisters earlier, but that's not solely on him.

I doubt they would fare well. One of the reason Pliny stays away from Rome is to avoid the conflict and conspiracies surrounding Nero. He'd probably do the same mistake as Gennai ("humiliating" Sadanobu) and incur the wrath of some higher-up (like when he yawned when Nero was singing). He's pretty close to Gennai in the way that they're both eccentric naturalists that travel the land. On top of that, he's also shit at understanding women, so I doubt he'd fare well in a world where they're the majority.
Lucius also seems too straight-forward to get involved in conspiracies. It could work with the right shogun like the guy in the first volume.

I feel like an American would almost never write "sure some people died but it worked" with absolutely zero self-conscious flinch. They really want the story to be that they had their cake and ate it too. In the fictional world red pox is so heinous that anyone would make that call so maybe I'm reading too much into it.

I have a surgeon friend that told me St. Jude's Children's Hospital is completely legit (if you're a foreigner, it's that they make a lot of sob story commercials to beg for donations, which can feel pretty suspicious) and the main thing they do that sets them apart is that they're willing to prescribe intense treatments normal pediatricians won't do because if they don't cure the kid it'll absolutely contribute to killing them. Normal pediatricians will often slow roll treatments that won't work but won't directly kill the kid either and just say there was nothing anyone could do when they die of the disease.
