In both anime and manga Joe savagely beats his opponent even after knocking him out of the ring but the difference is that in the manga Joe was fully conscious of what he was doing whereas in the anime he was blind with rage and wasn't fully in control. That said, the changes are small overall and I don't really see that much of an issue. I do hope that the inevitable remake sticks closer to the manga though.
