I would say Kishimoto always wanted to make Itachi have some hidden motivations behind his actions. But I'm pretty sure he didn't think his entire character through from the get go, considering how poorly written and retarded the Uchiha massacre was.

Itachi as a character goes to show just how deep Naruto is as a story. It's not just your average shonen.

I think he always pegged as secretly good. When he was fighting the other jonins and later Kakashi. He never once attempt to put the killing blow against them despite being obviously better and BTFOing them all. Even told them of they're from the Akatsuki and had a mission to capture Nardo when Kakashi directly asked.

The weekly mangaka's best friend is creating mystery boxes that they themselves don't necessarily hold the answers to. They throw out something vague but interesting to garner attention so that they can fill out the details in the future as they conceive of ways to execute on the initial idea.
