It's not until relatively late in fetal development that the balls and penis emerge from the body cavity for males. Prior to that, at least visibly there is no difference between males and female fetus. There has been more than a few cases where that process didn't happen and the poor kid and their family thought they had a bab y girl until puberty and out popped a penis.
