No. The browser downloads it for page load. Save just places a copy of the downloaded and cached file in a directory of your choice.

How does the browser download it for page load if I'm using the windows screenshot tool or an equivalent? I know this obviously doesn't keep people safe from downloading legit illegal content, that's a logical given. I'm curious as to how this isn't different than downloading an image outright though?

The browser needs to first transfer the picture to your computer to display it.
Press F12 to open the debugging view, network tab.
Press CTRL+SHIFT+R to reload the page.
Watch your image being downloaded in full.
