Ive been out of the loop with Spiderman, is it true the current Peter Parker is actually Dr. Octavius via body swap and the original Peter died in Doc Oc's frail old body???

That happened, but Peter's consciousness was actually inside his original body the entire time and Doc Ock never noticed. Eventually, Doc Ock sacrifices 'himself' to give Peter control of his body again and then he fights Green Goblin.

That was over a decade ago. Since then, Otto gave up and gave Peter is body back, then a backup of Otto's mind went from the Living Brain to a clone of Otto to a super clone body combining Peter and Otto's DNA, so Otto was the Spider-Man of San Francisco for a while, then he joined Hydra as the Superior Octopus, then he went back to being Spider-Man, and then the Status Quo SWAT team raided his house and now he's back to being Doc Ock because we all know change is poison in comic books and no one really wants it.
