How different would the Infinity Saga have been if this had been released instead of Captain Marvel as originally planned? Also, who would have played them over the literal whos they cast for the TV show?

They might have gotten a sequel but I doubt much else would have changed after the Fox deal aside from how 2014 Thanos was defeated

Inhumans was originally scheduled to be the final movie of Phase 3, so would have been after Endgame, it wouldn't have changed anything from earlier on in the saga. Vin Diesel seemed to be interested in playing Black Bolt.

If Feige hadn't dropped the Inhumans movie just because it was a thing Ike wanted, it would have been pretty much made before the Fox deal happened, and if they'd made a movie based on pre-2010s Inhumans, it would be it's own thing, in it's own niche, so "we have the rights to X-Men now" wouldn't really matter as they'd be completely different things. There's a non-zero possibility that Feige really would have been dumb enough to adapt 2010s Inhumans though.

I know but the deal would just be another reason as to why I doubt they would have used them much afterwards with the other being that I just don't think Feige likes them

Once you get to the point of them having a movie, whether they get used much afterwards would depend entirely on whether that movie performed well, not on whether Disney have the X-Men rights, when Inhumans and X-Men are actually completely separate concepts outside of a few years in the 2010s when Marvel tried to turn the Inhumans into knockoff X-Men and then the years when Hickman turned the X-Men into knockoff Inhumans but with pitfights and orgies.

And with the MCU as popular as it was in 2019, the worst case scenario for an Inhumans movie in 2019 would be only doing Ant-Man numbers, and that would be enough for then to greenlight a sequel.

I don't think Feige disliked the Inhumans specifically, it was Ike that he fell out with, and he didn't want to make an Inhumans movie specifically because it was the project Ike wanted to make. Captain Marvel and Black Panther had been the two projects Feige really wanted to make that Ike didn't want to do.
