One of the few reasonably good RPGmaker games and one of the few reasonably good Fallout parody games in general (even non-porn parodies).

The most unrealistic thing about games like TiTS and Lilith's Throne is that the player character is a member of the degenerate ultra-rich in a universe that is already degenerate compared to real life, but isn't involved in Epstein-like stuff.

And if you want to publish this game in some public manner, and don't want to go to jail, you don't make that content.
Then there's Japan, but that's a different story.

You know, for all the criticism CoC2 RIGHTFULLY gets, something it does much better than most other /d/ RPGs, including, or rather especially, "Battle Fuck" games is the victory sex scenes... by virtue of consistently having them in the first place. (Well, calling them "scenes" is an overstatement, most of them are just multi-page yapping sessions, but whatever.) Though, honestly this says more about the state of /d/ RPGs than about CoC2...

But more seriously games that have avoidable NTR are either games that are 90% NTR with a single (usually) short non-NTR route or games that are 90% non-NTR with a short NTR route that is clearly and blatantly labeled because NTR is scat-tier controversial. It's not really something you learn to love. Either you understand it or you don't. People who want to play a normal game don't generally want even the chance of NTR to happen. AA2 or RJ237060 might work if you're deadset on this I guess?

Leane of Evil Blade 1&2 might come close to what you want.

Both games have a prologue where you get cucked (though you can avoid it in 2), but the rest of the game is about waging a large scale war while also protecting your female generals and preventing them from being kidnapped and raped into submission by the enemy.

Wait, I almost forgot about Netoria Tactics: Revolution & Nebel Geisterjäger. Both have the threat of NTR, but if you play smart you can avoid it. Also there's decent content for holding onto your beloved.

DOL used to be fun, but the development has gone in the direction of pretending to be a Trauma Simulator™ with Deep Lore instead of the porn game about [definitely 18 year olds] being raped that it is.
Lilith's Throne is fun, but development is extremely slow and the developer keeps setting themselves deadlines that they then have to apologise for failing to meet.

There are difficulty settings. You can also do things like challenging the entire fortress and generally taking guns-blazing options for solving quests.

That's just weird to me. Once you've seen one, you've seen them all. At the very least NPCs could each have their own reactions based on some kind of personality system to actions taken, but it was all very primitive and too generalized to my taste.
It actually made me realize that I preferred the walls of text in CoC/2 more. Each time you sexed someone it was a unique "experience" (until you did it again, of course).

I will say it lasts slightly too long, but otherwise having a modular sex system like that is the best type you can have in any smut game. Is LT's take on it flawless? Hell no. But it still runs circles around 99% of other smut games. Which honestly says a more about the sorry state of actual sex in most sex games, but hey if you can't get prime rib you got to settle for the best hamburger.

Assuming you're the mod author, I only recently started playing but it's pretty fucking peak. Actually picked up Darkest Dungeon at a friend's recommendation purely for the mod.
My one sole complaint is that the futa doggirl that replaced the rabid hound gives your characters the beast kink but never fucks them. I frankly have no idea how complex DD modding is, but I'd cum buckets if they became grapplable if they had the appropriate kink for it.
Probably would be somewhat unbalanced to have a common and even repeatedly spawning enemy with some bosses able to grab players, but it'd be fucking hot so I certainly wouldn't mind
