SSRIs helped me (lexapro specifically), i also have ocd and white knuckled it for 7-8 years before saying fuck it and tried the pills
good decision honestly it solved a lot of my problems when nothing else would
Also had that issue for many years. As woo-woo as it sounds being mindful and trying to establish a mind-muscle connection went a long way. I was also 24/7 clenched, so the first step was deliberately laying down and remaining unclenched for as long as possible. Eventually, I could catch myself clenching when going about my day to day and stop myself from doing it. Lifting and running through the pain seemed to paradoxically help me. The doctor's initially told me I had epididymis, and said not to work out. Made things 10x worse.
I had great libido, good breathing, no tightness in the body but at one moment I lost libido, then few months later started choking and even had panic attacks
Now it's always right, stomach and throat too. I can run and swim without problems but it's hard to speak and breathe
Spontaneous hornyness disappeared, it's hard to get horny at all but morning wood is intact

I also clench my pelvic floor basically all day, perhaps not as bad as you guys as it seems to really effect you. But I do think it makes my dick smaller and contributes or stems from a mild constant sensation of anxiety. Im not a super anxious person but I would say I am tense for fucking sure.
i clench my pelvic floor when im edging and it stays that way, but yeah i guess pelvic floor exercises helps
