I know that it sounds insane since your a European, but poor people tend to live in places where access to cheap healthy food is terrible here in the United states. These places don't have grocery stores, the closest things they have are convenience stores like dollar generals and fast food places that sell nothing but slop.

Poor people don't have great transportation either. Convenience stores like dollar general purposefully set shop in rural and poor areas to outcompete grocery stores. Being poor is also stressful, so they have chronic stress and are always fat as a result. Poor people also often work multiple jobs, so they don't cook, and they don't learn to cook. The solution?
