They'll eventually calm down after a flare up to apologise, which clearly doesn't mean anything, and then just restart the cycle of being delusional and aggressive all over again, even when you try to deal with it and be there for them, it's quite sad and it's really a shame that it happens to otherwise nice people

I don't think detachment is strictly due to lack of empathy, I have had to detach from people like this, as hard as it might have been, for my own sake. I've often been too empathetic and understanding, even as much as for them to tell me that I was being too nice when they've had flare ups and how they don't deserve such kindness, but they continued to tread on it and never seemed to show any signs of change or real remorse, it's sad but time fixes it
