guy they interviewed on BBC said that boeing planes have an issue with the fuel switches being hit accidentally and locking, and that the FAA sent out an advisory asking (it was voluntary, not mandatory) airlines to check their switches
saars probably didn't get the memo, plus they took way too long to restart both engines after failure (10 seconds for the first, 14 for the second)
