miserable people quickly die out/are invisible in the third world. if you are neurotic in the third world, your chance of survival is very low. mentally ill people are unrecognized/misdiagnosed/mistreated. meanwhile normal people constantly live in the positive feedback loop of having to be extroverted, social, having a large family, having to go out to spend time and have fun etc. It's the perfect environment for filtering out vulnerable people and only letting healthy people survive.

but firstie misfits/outcasts/mentally ill are forcefully kept alive by the system. chance of being diagnosed is higher, therapists will feed you hundred chemicals to manipulate your behavior, suicide prevention measures are more common etc. they can also spend time on the internet instead of going out.
in the third world people like this would have been already honor killed by their family, crash out and get killed in whatever incident they chose to cause/get involved in, or commit suicide. first world however is designed to create and prolong the lives of outcasts and maximize their misery.
