Hair transplants are weird.

Some of them look fucked up and then the 5ar inhibitor medicine doesn't work and makes the hair look horrible.

And then some hair transplants look great and the medicine complements the transplant without any issues.

Seems kinda risky to me

The risky thing is finding the right clinic. There are hairmills that do 12 surgeries a day. You need to find one that has limits theirs to 1-3/day.
You can get sub-par techs do the angling of your hair wrong and overharvesting the back of your head.
