probably slang from a local dialect "cuk" which mean "fuck" thats also used as an expression of wonder, surprise, or even bro! at some point.
eg: gila cuk! (thats crazy!), cuk goblok lu! (damn you are dumb), cuk ganja! (dude! weed.) etc.
so macaroni cuck would be "dude! macaroni!"
in tongue of cheek manners if i had to guess
