Maybe if you had a heart attack at a really busy subway station in Tokyo or wherever, the average passerby wouldn't pay much attention. But in Japan they have loads of station attendants who tell you your bag is slightly unzipped or that there's ketchup on your t-shirt or whatever, they've got no real tasks to do most of the time so would absolutely love if someone had a heart attack
