Incredibly shit. Like some sort of earlier, even shittier somehow mini Russia they're a fucking kleptocratic basketcase of a country with theocratic shit making it even worse. They had a certain amount of irreplaceable lostech from an earlier age they managed to keep together with ever increasing amounts of bubblegum and tape, and slowly built up proxies and weapons horded over a long period. All of that has been pissed away or destroyed in the past few years so now they're left with effectively just what they can do domestically which is shit.
Make no mistake, they could still absolutely cause a lot of trouble, which is why many neighboring countries who fucking hate them, absolutely do not want them to have nukes, and are thus privately happy about them getting bloodied even while hating the isaelis too, are still at least publicly going "oh no naughty israel how dare you!" Like, if they targeted all their missiles and drones at Saudi Arabia's energy infrastructure that'd be real trouble and could disrupt a lot of production for awhile, which would spike global oil prices and in turn mess with the global economy. But that wouldn't ultimately win them anything, it'd just mean they got pounded even harder because the threat would be gone. They don't have a lot of cards that keep the status quo their corrupt evil elites want.

That's mostly because the air defense system calculates the trajectory of the threat and ignores it if it isn't going to hit anything. Stop coping. Iranian AD got disintegrates, various important figures vaporized and only thing Iran has to show for it are like 5 hits on random ass buildings.

Realistic: nothing. Israelis can hit them with impunity for as long as they like.
