The plan was an assault across the front to put some pressure elsewhere and keep the Union from concentrating on defending the objective. There was also a confederate artillery barrage that was supposed to soften the Union position. Both of these things fell through.
Bad assault, but it was already kind of a Hail Mary in a battle where Lee had overextended by even engaging. But thems the breaks.

Yes, although the Union took at least as many casualties and likely more (ACW casualties usually have to be given with a range of error). The Union ended up in a better position though so Lee withdrew. The entire battle was essentially a PR stunt on Lee's part, so anything less than a decisive victory was a failure.
