You don't understand muzzle flash. It's a combination of sudden expansion of gasses and combustible gasses and remnants of unburned powder reigniting once in contact with the atmosphere. It's would be extremely inefficient to have powder still burning out the muzzle, so the powder is tailored to a cartridge/barrel by making sure that you get a complete burn and then use the pressure still inside to keep applying force to the bullet. Unburnt powder is just a fact of life, and there will be gaseous byproducts that can combust but lack oxygen to do so (powder contains its own oxidizer). Once in contact with the atmosphere, they ignite outside the muzzle.
TL;DR - you can achieve complete powder burn and still see flash

Bore obstructions are unsafe because they bypass dwell time and can start early unlocking of the bolt, leading to case head failure and damage to the weapon. Banana-peeling the barrel is not as dangerous to the shooter, and doesn't magically cause bolt failure.

Not really. Shooting off a muzzle brake with a SLAP round will ruin your barrel, but it will not obliterate the receiver. And it's a known danger for spotters because the sabot pieces can be deflected towards people near the shooter. But not for destroying the bolt and receiver.
