Based on some evidence the brain has essentially a map of the body's sex that simply isn't aligned ( https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5738422/ mentions how it differs from homosexuality and https://en.wikipedia.org/wiki/Phantom_limb#Other_phantom_sensations is another consideration but these are examples ), and the disconnect creates a dysphoria. I call it a "sexed body-map in the brain", and because of the mismatch it creates something of a systemic condition (i.e. happening between multiple body parts).
