If brain scans show trans people are distinctly different than cis people, why isn't a brain scan part of the diagnosis for transgenderism? Why is self-reporting enough for one?

They dont show that. They show that on average there are differences that are seen in brains but also fmri studies are mostly all useless and dont really show anything useful.

In short, OP, LGBT as a whole doesn't really respect the practice of collecting data or using statistics or hard sciences to make determinations because those determinations are almost invariably going to be the opposite of what they want. They're basically a marginalized evolutionary dead end that has been given lots of political power and a platform to bitch at people with, and humanity does not fix broken people, we just let broken people stay broken and get new people, either by having children or importing immigrants.

LGBT doesn't really realize or internalize this however, and is still stuck under the delusion that all 8 billion people on the planet matter when there aren't enough tasks for them to do even in a society that is 75% pointless make-work, so they expect society to cater to their imagined needs and try to fix people who hate themselves. Society is not actually going to do that beyond the most shallow and vapid virtue signalling. There are reliable pheromone tests for sexuality (Savic 2005, Berglund 2006, Berglund 2008) but MRI scans for transsexuality mostly detect brain regions that are underactive due to dysphoria
A reliable hypothalamic scan for the BSTc, INAH3 and other nuclei would be a game-changer especially if it can distinguish binary MtFs and FtMs from theyfabs
