I'm currently quite fat (about 80lbs overweight) and been on HRT for a few years. I'm happy with the feminization I've gotten and the redistribution I had, but I was already fat to begin with, so the initial fat deposits didn't shift much.
Do I have to get super thin to get rid of my pre-HRT fat deposits? Does exercise play a role in this, or just pure CICO like in other cases?

fat deposits basically do not die. a fat cell has a lifespan of about a decade. what you need is a means of killing off specific fat deposits, and tightening skin. check this out for an example of how to minmax that process a bit.

to elaborate, there is no weight loss that will fix this. i spent 6 years consistently at my bottom out weight working a physically active job. ultimately my commitment to a low bodyweight over strong muscles cost me my job. when i came out of work, i very quickly returned to within 10 lbs of my upper weight. almost all of my fat deposits remained the same, save for some new ones acquired on HRT. in this time id made a decent bit of use of stimulants to try and push my body to fat cell killing weights but with no luck. fat cells have to be killed manually, diets wont touch them.

ive tried it in a couple different forms. i tried it topically to see if it could have local concentration related effects but that was a disaster, and i tried it orally. orally its great, 15mg is enough to slowly build new fat deposits, and tends to put more energy in my muscles when im working out, which i strongly suggest you do in order to support the creation of lower body mass. i also use low dose oral albuterol to assist with weight redistribution and stimulant effect on exercise days.

fat cells dont die on their own in practical terms. the only people ive ever heard say they do have never had to contend with them. they do have 2 methods of dying via external intervention however: apoptosis, and necrosis. apoptosis is where the cell is triggered to kill itself, releasing its contents in a controlled manner to be cleaned up by the lymphatic system. necrotic death involves uncontrolled cell rupture, results in far higher inflammatory signals, and takes longer to heal.

depending on the method, apoptosis can be more or less preferable. phosphatidyl choline injections are a common method of triggering apoptosis however its possible for cells to become resistant to them. cryolipolysis is another common apoptotic method of killing fat cells which exploits their heightened vulnerability to death from lower temperatures vs higher temperatures. there ere certain enzymes you can get in injectable solutions that also claim to trigger fat cell apoptosis.

aqualyx (deoxycholate solution) injections are a common method of destroying fat cells via necrotic methods. they are impossible for fat cells to resist damage from, but their necrotic method of action means they take between 1 and 2 months for swelling to go down from.

i do have some thoughts regarding weight gain after actually. in my experience whats killed with deoxycholate will not come back very easily, but PPC injections seem less capable of producing sustained loss of fat in an area. this is likely because deoxycholate doesnt just kill fat cells, but also fat cell precursor cells, preventing regrowth with weight gain to a degree. i also suspect fat is typically gained preferentially in loose areas of skin, so getting skin tightening treatments like HIFU or Fractional RF microneedling may be a good idea to gain lasting alterations to your shape. all of these treatments are available at low costs from aesthetic clinics, with a HIFU facelift costing just £300 in some UK clinics. be aware that these also destroy fat cells to an extent too, so do not use them in areas you may want to gain volume like the upper cheeks.
