Picrel is me at 3 years HRT. My BMI is 24. I exercise 3-4x per week. i wanted to document some important effects i didn't know when i started:
>Strength
you don't just lose muscle mass or volume on HRT you lose "fast twitch" muscle fibers, those necessary for explosive strength, Particularly in the upper body. as you age on estrogen in your 30s and 40s your body literally strips down these muscles while preserving subcutaneous fat in the lower abdomen, thighs and buttocks. You will get T Rex arms if you do not train them. Also: my average run speed dropped by 32% and my endurance went down by almost 50%.
>gym/weight loss
You probably do not have the time and energy to keep an athletic figure on estrogen: you can lift and run as hard as you want but building muscle will be much much harder. Meanwhile losing weight will come out of your upper body first, resulting in the "pear" shape. People will tell you to take pio and weight cycle. Do not do this! You will develop visceral fat if you upcycle too fast and on the down cycle you will lose muscle as well as upper body fat. Just let nature take its course, do not rush it.
>APT/posture
a sedentary lifestyle, high heels, and of course estrogen can lead to a weakening of both hip flexor and abdominal muscles that produce anterior pelvic tilt and a convex belly. Mitigating this takes time and effort out of your day (i.e. yoga) and it gets harder as you age- your muscles get tighter and your abdominal ligaments stretch out.
>sexual function
My T was nuked and my testicles removed early in transition. Despite regular manual stimulation, my erect size fell by almost 50%, and I developed erectile dysfunction. I can ONLY achieve erection while supine/prone with difficulty. I can no longer have a satisfying orgasm without anal penetration. Some will tell you these things do not happen, but they do. Your sex life will need to change on HRT as will your relationships.
