This decides whether you develop ovaries or testes
But sometimes you develop testes and they make testosterone, but testosterone doesn't work on you and you're born female. Male technically, yes, but you will never pass as one.
Sometimes for whatever reason your body can't pick a direction and you have both ovarian and testicular tissue. This can happen even if all your cells are XX, and in fact XX is the most common karyotype when that happens. Decent chance you'll be assigned male if this happens, and all bets are kind of off for what happens during puberty.
This is without even touching intentional meddling with hormones, but the OG youngshits were intersex kids anyway.
