For people with gender dysphoria, these surgeries are not generally conducted under the age of majority, unless the patient is both intersex and trans, because society deems the surgeries to be appropriate on intersex people for some unknown reason.
We are instead discussing hormone replacements. In both cases these are not cosmetic but rather biological realignment operations. It biologically realigns the deeply felt and intrinsic sex-identity with the bodily sex.
