'butch' is an identity. Same deal with 'femme'. They came up out of the pre-stonewall lesbian bar scene. They are not some aesthetic descriptors.
I think what we have seen over the past decade or so are a number of femreppers transitioning, paired with the butch/femme identities falling out of favor with zoomers. That combined effect is leading to butches being far more rare than they were previously.

I don't think this is helped by the damage that bis have done to lesbian culture over the past several decades. It is hard for specifically lesbian identities to exist in a sea of bisexual 'sapphics' who demand inclusion in all instances. They are the same group responsible for 'butch' and 'femme' being reduced to the point where people just think they refer to aesthetics or that you can be a 'bisexual femme'.
This cultural vandalism is an argument for better gate keeping and a degree of separatism desu.

Literally read up on the history of the identities and the lesbian bar scene. Femme and butch are and have always been exclusively lesbian identities.
In the 70s you did have a divide between bis who wanted to either tear down lesbian culture or hijack it for their own ends. The natural response to this was separatism. At this point, returning to separatism to with avengeance seems more than necessary.

The term in a casual environment used to be inclusive of bis. But never formally or in a cultural context. Certainly not when it comes to talking about actual lesbian identities and subcultures.

No. Many 'mascs' are bisexual. Butch is an exclusivity lesbian identity. Same deal with femmes; Plenty of feminine bis, but the very idea of a bi-femme is an oxymoron.

Honestly, this is the sort of bi historical revisionism and lesbian erasure that really makes me sympathetic to the gold star crazies. Bis clearly do not want coexistence and shared spaces, they want assimilation and extermination.
