I feel like the influx of autistic trenders/perverted men have destroyed the image of trans people and now genuine dysphorics are seen the same as someone that puts 0 effort into their transition while talking about their "princess wand". if doctors could actually be trusted would you be more open to transmedicalism?

I'm autistic, fetishistic, and I still have dysphoria. I'm an ineffectual introvert boymoder & I don't ever hurt trans optics because I never declare myself trans IRL or online (except here), plus I put a lot of effort into my transition. If I were to be gatekept just because of my autism and fetishes, I'd be miserable and deprived of the medicine I need to be functional (I've tried to detrans, it went terribly.)
tldr: Autistic fetishists can be dysphoric too.

you cannot medically gatekeep someone from putting on a dress and demanding she/her pronouns. amazon basics skirts are not ever going to require a fucking prescription.
you can medically gatekeep someone from taking hrt, with some degree of effectiveness. DIY is of course possible but that requires agency a lot of people don't really have, and governments have tried to prevent it with varying levels of success.
when medical gatekeeping exists, it has generally included RLE requirements that are practically optimized to make people hate trannies, by forcing trans people to be openly trans in potentially hostile environments.

at this point all of this should be obvious, and i have a hard time understanding how people don't see it. if you're worried about optics the informed consent model is your best friend, at least for estrogen.

gatekeeping as in any medically diagnostic procedure isn't that bad, most of the issues with it come from the fact that clinics usually run a monopoly in their area
that leads to excessively long waiting lists, humiliation rituals, and a vulnerability to biased research output
if it became part of standard care and the psychopathologization of gender dysphoria was recast as a natural response, then most of the issues would dissolve

This is a good point, but unreasonable in any form other than an informed consent model, as expecting ALL general practitioners to be able to complete a sexological assessment is unreasonable.
