Start with an outline. If you start by trying to bang out the whole thing you'll realize by page 5 that you have 300 more pages to go and get overwhelmed. Building from the inside out helps to give you a sense of completion for each task, as well as it helps keep your thoughts organized and thus makes the final product more clear.
