This is also my hangup. I find that "rephrasing what is already written" is the worst thing you could do, despite it being the method almost anyone who has ever attended primary school was taught to do. If it's already in the book, it's not worth rewriting, you're just going to write a shallow imitation of it. Instead, I write my fleeting reactions and questions, things that are not captured in the text, but may provide clues for understanding once I reference them later. Dumping stream of consciousness onto the paper as a future aide memoir.
Also, if what you're writing is not worth returning to in the future, it wasn't worth writing to begin with.

The only people who do this are college students or women who want to appear intelligent/quirky. You should not be annotating your books. You should be reading intently, a bit slowly, and hanging onto words so you fully understand the scene going on.

To not take notes is to passively consume content and just letting ideas pass through. I've actually tried to force myself to not take any notes, and it turns out I will use my brain to memorize all the thoughts that I come up with. Quite a stretching.

Sometimes, but mostly I do very quick rereading immediately after finishing for the first time and that's when I take notes. I use notepad++ for that.

Sure do. They always help me form a cohesive view of the whole book after I've finished reading it. Also the notes aren't just strictly limited to what'll help with the "book report". It ranges from new words to mini bibliographies.
