It was a combination of things. No doubt he played up the benefits toward his health issues of using heroin so as to have an excuse to keep being an active heroin addict, but I do believe his health issues were real. He had obvious autism or neurodivergency of some kind, going by his journals, general childlike/immature manner, prexisting mental health issues and so on, and I wouldn't be surprised if today he'd have been classed as part of the common autism/adhd/hypermobility trifecta and that that's why he was prone to posture issues fucking his joints and digestion issues.
