did this start when you replaced the tensioner?
are you 100% sure you're routing the belt correctly? check a diagram carefully and make sure the diagram is for your exact engine.
did you compare the old and new tensioners to make sure there is no substantial difference?
are you sure the new tensioner you put on is seated correctly and fully against its mating surface?

all cuz i switched out the tensioner i bet. i thought i was doing preventative maintenance but caused all this shit. last time i thought it was because the water pump seized so i replaced the whole thing. But it was probably this the whole time. Either AC delco gave me a piece of shit or i installed it wrong.

maybe it's confirmation bias because i don't know what vehicle this is but it looks like an obs ford? i had this problem and it ended up being the water pump. it was still working but the something was wrong with the impeller and it would get stuck or jerk or whatever the belt would move just a little bit. if you just watched it running it seemed fine and i only noticed it when i looked one day after replacing it for the 3rd time and it was only half way still on the water pump pulley.

can you get the old tensioner out of the trash or something so you can compare them?
another option is take out the spark plugs, put on a new belt, and have someone run the starter while you watch the belt closely, is it catching, rubbing, or not seated properly somewhere?

no I threw it away like an asshole. i replaced that one because the old belt squeaked. I thought that was gone. I replaced the old one and it got shredded after a month. I replaced this one and it lasted me 2 months? then it slipped off while under acceleration at like 2800 rpm. stupid shit.
