A lot of foreigners tend to think of Italy as one single block from north to south, but that’s really not the case, the north and the south are as different as night and day. There are definitely mafia-type organizations that originated in southern Italy, like Cosa Nostra in Sicily, the ’Ndrangheta in Calabria (which is actually the most powerful), the Camorra in Naples, and a few smaller ones. These groups have a big influence mostly in the south, where they’ve gotten into local politics, public contracts, the economy, government institutions, and sometimes even into the police, judges, and politicians. So yeah, organized crime has a real presence in the system, but not to the extent that you could honestly say the whole country (north to south) is run by the "mafia".
