In the early 60's NASA had a science problem. By the mid 60's it was more of a safety problem. The three astronauts that died were really more the result of a lackadsical safety culture at NASA and it's primary contractor. Once that began to improve it paved the way to getting people on the moon.

The reason for this is well known. NASA started as every government project where large parts of the work was contracted out to low bidders. After the fire incident everything was done in-house by NASA where they had the ability to inspect the quality of the work at every step. The fact that NASA was mainly competent white guys at the time rather than being a DEI dumping ground as it is now didn't hurt
