The issue is that Iran is at war with Israel and neither side has the ability to conventionally fight the other, making missile and air strikes the only real options. However conventional bombs can't defeat either country meaning they might have to use nuclear bombs since nukes dramatically increase the explosive yield and that might be whats needed for either side to win.
There is also an issue of not knowing what armaments are on the missiles coming at you and still having to decide whether and how to respond when detecting a barrage. With the US and USSR, a dozen missiles being sent either way, even without nuclear warheads, could've resulted in nuclear retaliation on detection. However the US and USSR are a fair distance apart so detection system give a somewhat large window of time to decide on what to do
With Iran and Israel, with missiles at hypersonic speeds, the window of time to decide what to do is much smaller which might make either side decide to launch a retaliation strike with nukes before the other side's missiles have hit.
