At least in the U.S., the majority of STD infections are in homosexuals. I'm not even exaggerating. Homosexual men are over 50% of HIV infections even though homosexual men are only about 3% of the population.

Create an HIV registry similar to the sex offender registry. Make it illegal for anyone with HIV to have sex with anyone without it. Send violators to an HIV colony from which no one may ever leave.
You'd also need to test all immigrants. If they're positive send them back. If they infect anyone don't even send them to the colony, just kill them.
