That's a long distance but in some of the ads for the bombs they depict them as being thrown down a shaft in the bunker, which makes a lot more sense than trying to pierce a mountain with a hunk of metal.

Probably. Bombs and missiles aren't 100% accurate like you see on the TV. It's even harder to get precise hits if there are enemy air defenses in the vicinity of the target.

they are, the deviation range is 10-50 m max, even for dumb bombs atm like FABs. the 14 ton bomb might be more, but surely not 3km away from target, plus its designed to bomb specific ground location so should be pretty precise

miss can be if GPS is being highjacked, ergo most US missiles and bombs in Ukraine, but i`d expect for this operation it to taken in account. So far i believe they hit what intended and it was entrances to facility (there are 5 in different directions, they bombed one). Effect of it? Probably none.
