This is not generally true anymore. In the 90s around half of hospital (infant) circumcision in USA were without anesthetic, but the standard of care these days is penile nerve block with lidocaine injection.

It is so fucked though. I read an American Academy of Pediatrics paper where they cited studies where they performed circumcision with and without anesthetic and literally measured the screams of the infants to judge what was most effective.
