They are just not directly. The hospitals have to charge paying customer enough to cover the freeloaders. The insurance companies are then billed the inflated amount, and negotiate it down to a lower but still idiotically high price. In general the insurance companies don't care that much because their profit margins are not dependent on the amount they are billed. If they are charged more, they make more because so much of the billing is done as cost-plus or a fixed percentage of the amount billed. In the end, they WANT to be overbilled, because they make more money that way. This removes all downward pressure on pricing the services and procedures, and everyone in the healthcare business gets rich. Look up what traveling nurses make in the US right now.

What actually happens is that illegals and people with zero coverage are constantly going to hospitals for treatment, and the hospitals treat them because they are required to by law.
Then when those bills go unpaid, the doctors and hospitals have to write off those expenses and increase what they charge the people who do have private insurance or Medicare/Medicaid. The fact that so many people do have private insurance or government coverage allows these charges to spiral out of control, because the jews at the insurance company will negotiate down the most egregious charges and then pay the rest.
Most of the sob stories about people going bankrupt because they got screwed by their insurance company or their hospital bills are total horseshit.
For anyone who has private health insurance, there is a statutory maximum out of pocket limit of $9,200 in 2025. Even if you get turboAIDsCancer and had insurance with the highest possible deductible, you won't ever owe more than that unless you do something completely retarded.
Almost $10k is a lot of money, but it shouldn't be the kind of thing that leads to homelessness or whatever. Keep in mind that's the absolute worst case scenario. And doctors and hospitals will be happy to negotiate payment plans or write off part of the bill if you really can't afford it.
The people who end up with the hundreds of thousands in medical bills usually either chose to not actually have insurance and then caught turbocancer, or they insisted on getting unnecessary/elective medical treatments despite their doctors and insurance company telling them that they don't need it and shouldn't get it and it won't be covered by insurance.
