In an official statement, Captain Sam Thomas, President of Air Line Pilots Association (ALPA) India, said, "We feel that the investigation is being driven in a direction presuming the guilt of pilots, and we strongly object to this line of thought."

The association expressed surprise at the “secrecy surrounding these investigations” and questioned the qualifications of those involved. “We are reiterating the fact that suitably qualified personnel are not taken on board for these crucial investigations,” the statement read.

The pilots’ body also flagged a Wall Street Journal report published on July 10 that referred to the inadvertent movement of fuel control switches — a critical issue highlighted in the Aircraft Accident Investigation Bureau (AAIB) report. “How did this information reach them?” the statement asked, referring to the article.

The AAIB’s preliminary findings revealed that both fuel control switches of the Boeing 787 aircraft were flipped from “RUN” to “CUTOFF” within seconds of take-off from Ahmedabad on June 12, leading to engine shutdown. The cockpit voice recorder captured one pilot asking, “Why did you cut off the fuel?” and the other responding, “I did not do so.”

A new theory is that the pilot who asked why did you cut off the fuel was actually the one that did it and was framing the other pilot on the cockpit voice recording. This way his family can always believe it was the other pilot who killed them.

It was not at the same time. There was a separation of 1 sec between which is consistent with one person using one hand to flip one switch then the other.

A newly released preliminary report may finally shed light on what went wrong. Spoiler alert: it's great news for Boeing. Early findings suggest the mid-air disaster was not due to a mechanical flaw in the engines, but likely the result of human error.

The report does not explain why the switches were flipped and points to no mechanical issues or bird strike. It does, however, suggest a focus on the pilots' actions. The first officer was flying at the time.

As we've previously noted, this marks the first-ever hull loss of a Boeing 787 Dreamliner since the aircraft entered service in 2011.

At the Paris Air Show, one of the aerospace industry's biggest events, held about a week after the crash, Boeing kept a low profile while the investigation into Flight 171 was ongoing.

Shawn Pruchnicki, a former airline accident investigator and aviation expert at Ohio State University, told The New York Times: "For example, on the 787 and probably more airliners these days, the switches themselves — you can't shut them off without actually lifting them up. So there's a little mechanical gate built into the switch — you have to lift it up over this little gate. So you can't just bump it."

The good news for Boeing is that the investigation is pointing toward human error as the contributing factor in the crash.

Spool up time, spool down time, time for the last bit of fuel to squirt through the lines & injectors. Engineering does not assume suicidal pilots in any analysis. Because you can just shove the aircraft attitude nose down and crash whenever the fuck you want. Amongst thousands of other options in these cockpits that lead to suicide.

We protect against single fuck ups. Not dual fuck ups and suicides. These were protected, locked switches. You have to deliberately move them to do the action. A bump won't move them, they literally survived a CRASH in the same position they were left in.

Incompetence can be protected against with training. India is always suspect for this due to their scam culture, but they probably take aviation pretty seriously because the US makes them behind the scenes. So this is probably suicide.

Unlikely. Crash report shows a photo of the center console recovered, switches were exactly where they were supposed to be per flight recorder. Bad switches would be in the down position (unless they messed with them) for the photo.

The wrecked switches will definitely get the full monte destructive inspection for the final report. They will be sure to rule out any and all causes from mechanical switch failure since it is central to this case. If ANYTHING is found, expect 787s to get grounded until the issue is fixed.

These aren't standard switches like you have in your light switch at home. These are held in place by a spring and have a little gate you have to jump. So you must pull the switch out, transition it, then let it go to change it's position. You can bump it all you want, it won't move. It also has switch guards to stop crap from catching on it and giving them a yank.

Additionally, these switches survived a crash locked where they were at impact.

No amount of poo would move those switches. In fact, poo would have made it IMPOSSIBLE to cut off the fuel, because the switch would be too slippery to grip.

Because the engines were running. The way the spring loaded interlock is designed you cannot put the switch into a position that will activate fuel flow and allow the engines to start without the interlock activating and preventing the switch from moving from the on position. It could not have been moved accidentally. The black box telemetry showing that they were shut down one after the other with a 1 second interval between them further reinforces that it was a deliberate act. It subsequently registered a 2 second gap between reactivation of the switches as the pilot was under durress trying to control the plane while reactivating the fuel system.

Nepal probably has the worst aviation safety record in the world, India probably has the second worst.

The issue is basically that Indian pilots can't be prevented physically leaving India, so are constantly defecting to other counties where pay is far better.

You have Indian and American pilots landing on the same air strip one after another, but one is getting paid a quarter of the other. American pilot training is undermined by cheap jeet labour, quality of jeet services are undermined by exodus of the best staff.

These aren't Indian pilots, they're the pilots left in India. They suck. The second an Indian gets good at anything they try to leave India.

And you can probably see, this issue would also apply to avionics technicians, ground control, electrical engineers, it compounds. The problem is lack of notification when the switches were flipped off. Immediate switch back on might have made the recovery possible and asking questions unnecessary. The jeet in charge didn't know what the other jeet was doing. No notification for cutting engines off during the takeoff sounds ridiculous. Whoever designed the system didn't consider loss of power in both engines urgent enough and forcing the time waste.

I think what he means is that the co-pilot should have taken action to restart the engine without hesitation as soon as he noticed the problem.
He should have declared that he would take over the controls without asking any questions, though that might have been a bit harsh but that was his role.
Even if that might not have been enough to save the plane, it could indicate the pilot's lack of experience, lack of training in emergency response, or communication issues within the cockpit(CRM).

The two switches are like the reverse on some cars. E.g. on some older vehicles you need to lift the stick or hold a button before you can stick it into reverse. The fuel cut off on the Dreamliner is similar whereby you need to pull to switch, shift it to the side, pull it down and drop it into the off position. Twice. One for each engine. It isn't something you can just do by accident. So this makes me think there are possible 3 causes of this clusterfuck:
1. Tired, dumb poo co-pilot for some reason did this twice by mistake during take off
2. Broken poo decided to suice everyone
3. There is a flaw in the dreamliner which has caused both engine fuel supply valves to close with zero input from the pilots. When the switch is activated or deactivated, it tells the EEC ( electronic engine control) to open / close the fuel supply valve. A bug in the EEC could have decided to close the fuel supply itself.

I am no poo apologist and as fun as it is to assume death-by-poo ineptitude, I know Boeing and I'm thinking #3. Until, which I doubt they will since everything is scrap metal, they find the panel with those switches down, I think another case of Boeing fucking up and covering there arses. A company who has had whistle-blowers assassinated.

Hard-starving the engines instead of going through a proper shutdown sequence will leave pressure in the system that will keep the engines going for a while. That's assuming he was aware what had cut off fuel to the engine. In this case it appears to be manual input.

What it looks like is one of them turned off both lines simultaneously, the other pilot saw power drop and asked the other pilot what he did, then see's the switches off and then flips them.

A bug that causes the fuel supply to close to both engines, but delayed by the exact interval you would expect if the switches were manually actuated? But that means the physical switches on the console would still be in the on position. Did they have to cycle the switches off then back on? Why is a second off actuation not present in the telemetry, or are you implying actuations are recorded from hardware other than the physical switch itself? A physical issue like that seems extremely improbable, especially considering the investigation is leaning toward a pilot issue, rather than putting out an emergency TSB for those switches as is the norm across the industry even in events where there are no injuries or fatalities. There was a big TSB put out for some of the engines the max uses as well as one of the comparable airbusses last month.

Even with the 3 pilots in cockpit rule after germanwing incidence is a lukewarm attempt to address the issue. JAL 350 crashed the with 2 pilots cockpit fighting the rogue pilot trying to crash the plane, though they managed a more "controlled" crash into the water that only killed 24 people instead all hands on deck.
