It's real but if they tried it, they'll maybe hit 10-20% of their targets, mostly in the Middle East. The rest of their targets in Europe will be intercepted by missile defence, followed by a barrage of retaliatory nukes in accordance with MAD. Israel gets glassed along with parts of Syria and Jordan as collateral damage, emergency conferences follow to stop further escalation, things will be tense for a few years but the world will move on.
