Medicaid comes largely from the Feds, but a lot comes from the states. When it comes down to it, it's the sanctuary states that pay into medicaid more than most. The exceptions might include Texas and Florida.

I'm fairly sure that non-sanctuary states either don't really mind or would be very glad to see less illegals flooding the system and making them look bad.

The message is closer to Trump putting a knife in the hands of governors and letting them make unpopular choices just to buy votes from disloyal foreigners.
