Seems very unlikely. McDonald's is a franchise, so it's not like the worldwide company takes a hit for the free meals, it'd just be whoever owns the particular restaurant.

Bill Gates makes $80 per second. Most McDonald's staff and managers probably wouldn't recognize a gold card, and would spend time discussing and validating it somehow. The free meal isn't worth his time, unless he gets some kick out of flashing his gold card.

Why the hell would he eat at McDonald's anyway? To cultivate some common man appearance? Using an ultra-rare perk would spoil that attempt.
