Heavy drinking areas in osaka and shibuya can get a little dirty but they are cleaned up by 7am.
Why do you think our trains stop at midnight? To clean. We believe in staying clean, and try to be somewhat minimalist.
Also, we actually recycle. We burn most trash, and generate power from it cleanly. We use solar and hydroelectric, and more.
The main reason why it works is population density, however. There is a critical mass in each city that can support it.
Many historic castles and sites crumble because some wards and shrines don't have many visitors.
We do have less children. It is an issue but there was less people in showa era too, and things were fine. Most trains were built in the 80s, actually. Still work great.
