It's amoritized capital investment in the machinery (which the overwhelming majority of doctors had no part in designing, building, or even conceptualizing) for testing and processing, along with pharmaceutical overhead. There's no reason doctor's can be highly paid in a national system, though it's possible there would or could be some trimming. But I'm skeptical it would hit fields like cardiology, neurology, or oncology particularly hard.
