not sure of the cause but the doctors might have actually put the maggots there. its called debridement therapy. the holes are probably just untreated ulcers full of dead flesh. the maggots eat the dead stuff but leave living tissue.

Maggot debridement therapy is done rarely on patients and after primary care. Notice how the patient is dirty, his wounds are putrefied, oozing pus, he's uncleaned. That's not maggot debridement therapy, that's a homeless person who has been just rescued from deplorable living conditions.
