I've been collecting the relative autopsy and toxicology reports of deceased female pornstars since sometime early last year. I usually write to the coroner's office of the area that they past away asking for the autopsy and toxicology reports as part of my graduate research program and I need them for my thesis. This isn't true, but sometimes I say this if they ask what I want it for. Either way, I'm still well within my legal rights to acquire these documents. Sometimes I'll get a police report along with the autopsy and toxicology report.

So far I've collected:

Dakota Skye (Acute multidrug overdose)
Dahlia Skye (Self inflicted gunshot wound to the head)
Jessica Jaymes (Seizure and chronic ethanol abuse)
Olivia Nova (ethanol abuse and recent cocaine abuse)
August Ames (Asphyxiation from hanging herself from a tree in a public park)
Amber Rayne (Cocaine intoxication with alcohol abuse)

Kylie Page has just past away too (pic related). Still under investigation, but I've made moves to acquire the autopsy and toxicology reports.

If you want to know why it's because in a strange twist, acquiring the autopsy reports of deceased pornstars actually breathes life into them for me. My gooning had reached critical mass and what little humanity was left between my degenerate, one-dimensional relationship and the pornstars I was cranking it I was able to restore quite successfully by reminding myself of their mortality. The whole process is very respectable and methodical. I enjoy organising, printing and filing them. Sort of the opposite of what you do when your jorkin it to porn, funnily enough.

I do. She had no ethanol in her system, so she wasn't drunk, and she wasn't on any antipsychotics either. Benzoylecgonine was detected (not a benzo), it's a metabolite of cocaine. And cocaine itself was also detected (yet to be broken down into metabolites).

She had methamphetamine in her system, and amphetamine because regular amphetamine is a metabolite of methamphetamine. They also picked up MDMA and MDA (on the other page not in this screenshot).

And fentanyl, which is probably the main culprit.

I haven't been outright refused yet, but I have ran into a lot of trouble locating and retrieving the relevant reports for Jesse Jane. Either I contacted the wrong coroner's office or I had her place of death wrong, or her name wrong. Might try again at some point. I think more information is out now probably.

Another thing I do is go and read their twitter to see what they were posting about in the days leading up to their death. Kagney had several posts about not using drugs and being straight edge. Her toxicology report only showed evidence of nicotine and cotinine (found in tea). She was being honest, and her decision to end her life was not cooked up in a haze of drugs too.

Some of the reports are free, some require a small fee (usually about $30). So they're not available online on a website, but can be obtained by anyone with an internet connection and a debit card, really.

This is a pretty massive question. Lots of first world countries abuse drugs, it just so happens that North America is geographically positioned to get pumped full of fentanyl in addition to being the porn production capital of the world. So at least in part the answer is really just because that's where all the pornstars are and that's where the most fatal and easiest to overdose on drugs are.

The other reasons I have are a bit more personal opinion and nebulous / superstitious. I'll try not to drag on but more or less this is my take: Partying and taking drugs are for people who either have nothing else or can't do anything else. Every single pornstar that's "made it out" was eventually gripped by something that they thought was better, more interesting, more fulfilling, etc than partying, taking drugs and getting railed for cash. Anyone ever will party and do drugs and have sex forever until something touches their spirit that they desire more than a life that's nothing other than running on a hedonistic treadmill over and over.

There's obviously other factors. I think one of them had cancer or some serious illness. Sometimes they're experiencing psychosis and not in their right frame of mind. Deceased female pornstars are obviously very young and in a pretty bad scene that definitely fucks with their heads. It's hard to imagine them dying in any other way other than overdose or suicide, really.
