My acne is persistent especially on the sides of my face but I can cover it up most of the time. I have a lot of dark spots on my forehead, and I also have a big forehead which I can hopefully get forehead reduction surgery on.

That's bad luck on both counts. A lot dose mood stabilizer might help with the paranoia, but therapy is a better long term option. Being honest with your future bf about your paranoia might help too because he can help you rationalize your feelings, but you'll need to trust him a lot for that.
